What is Health Home?
Health Home services provided by a Care Coordinator focus on:
- Improving the individual’s quality of life
- Helping to manage multiple providers
- Assisting with getting appointments
- Identifying helpful community resources
- Connecting to available benefits
- Helping with post-hospital care
Who does Health Home serve?
The Health Home program serves Apple Health clients of all ages, including Medicaid/Medicare dual-eligible clients approved by The Washington State Health Care Authority. Individuals may be eligible if they have one identified chronic condition and are at risk for a second chronic condition. Contact your Apple Health plan to find out if you qualify for the Health Home program and if Compass Health is contracted to provide Health Home services for your plan.
Compass Health delivers Health Home services in the homes and communities of clients in Snohomish, Skagit, Island, San Juan, and Whatcom counties.
Approach to Care
Health Home Care Coordinators work to reduce gaps in services, increase coordination of all service providers, and empower clients to take charge of their own healthcare. Partnering with clients, their families, and providers, Health Home Care Coordinators work to ensure coordination across systems of care, including medical, behavioral health, long-term services and supports, and other social services.
Goals of Health Home
The Health Home program aims to assist individuals in achieving optimal health and well-being by improving coordination and quality of care while also increasing their participation and ownership of their care.
