Non-Medicaid Transitions Program (Snohomish County)

The Snohomish County Non-Medicaid Transitions Program is focused on assisting individuals who do not have Medicaid or other methods of paying for services to stabilize after a psychiatric hospitalization, incarceration or other crisis episode. Individuals are provided with case management services as well as brief counseling and the ability to meet with a psychiatric provider if medically necessary.

The program is short-term and focused on stabilization and connection to community resources, to include application for benefits that might provide the opportunity for stabilization and increased health and wellness.

To be eligible individuals must be in one of the aforementioned transitions, live in Snohomish County and have no means of paying for services. Contact us for referral information.
