Jail Transition Services
Through our Jail Transition program we are able to provide short-term transitional services to adults transitioning back to the community from jail. This program is only available in Snohomish and Island Counties. Individuals will be referred to this program prior to leaving the jail. Services provided include assessment, comprehensive community support, assistance with obtaining financial entitlements and housing, medication monitoring, referral to needed resources and linkage to ongoing to ongoing mental health services.
