Projects for Assistance in the Transition from Homelessness (PATH)
The PATH program provides outreach to homeless people on the streets, under bridges, in shelters, and in other non-traditional settings. Program staff screen individuals for needed services including mental health, substance use, medical, economic, and other community services. The program also assists in the identification of housing options. The program provides transitional pre-treatment community support services, referrals, and linkage to needed ongoing behavioral health treatment and other community resources.
