Program of Assertive Community Treatment (PACT)

The Program of Assertive Community Treatment (PACT) is a fidelity based intensive outpatient treatment program designed to assist adults overcome the barriers to their recovery from severe and persistent behavioral health disorders.

The program operates with a strength-based and person-centered approach and is targeted to individuals with severe behavioral health disorders who exhibit continuous high service needs, who have experienced multiple inpatient psychiatric hospitalizations, and who have been unable to benefit from more traditional outpatient behavioral healthcare.

PACT services are provided by a multi-disciplinary team that includes mental health professionals, a psychiatric prescriber, nurses, a chemical dependency treatment specialist, a vocational specialist, peer counselors, and community support specialists.  The PACT team carries the caseload rather than an individual clinician.  The majority of services are provided in the client’s home or other community settings and clients are seen several times per week in order to promote stabilization and recovery.

Services include comprehensive assessment, individualized treatment planning, service coordination, counseling and psychotherapy, co-occurring disorder treatment, vocational services, comprehensive community support, peer support, assistance with daily living activities, crisis intervention, and psychiatric evaluation, prescribing, and medication monitoring. Services are provided seven days per week including weekends and holidays. Crisis services are available 24/7.
