Community Transitions
- Transition & Engagement Services
The Compass Health Community Transitions program works with individuals who need connection to resources to improve their health - resources including behavioral health care, physical health care, and resources to address social needs such as shelter/housing, food, and transportation. We have found one of the best ways to reach individuals who may be in need of these services is through community emergency services providers such as EMS/paramedics and law enforcement. Together with our law enforcement and fire department partners, Community Transitions care coordinators and community health workers conduct outreach and engagement to individuals at a variety of locations in the community to assess needs and provide ongoing support to assist individuals in getting connected to the most appropriate pathways to address their needs. The Community Transitions program is supported by several partners in south Snohomish County, including:

Recovery Navigator Program
- Substance Use Disorders
- Transition & Engagement Services
The Recovery Navigator Program provides community-based outreach, intake, assessment, and connection to services and, as appropriate, long-term intensive case management and recovery coaching services. Individuals with substance use disorders (SUD) or co-occurring substance use disorders and mental health conditions are referred to the program from diverse sources. Recovery Navigator staff shall facilitate and coordinate connections to a broad range of community resources for youth and adults with SUD, including treatment and recovery support services.
Eligibility
San Juan County residents, youth or adult, are eligible. Eligible residents may have Medicaid, commercial insurance or no insurance. Typically, clients referred by law enforcement, community agencies, and court system; however, all referral types are accepted. Learn MoreAdult Intensive Outpatient Program (IOP)
- Adult Outpatient Services
The Adult Intensive Outpatient Program serves individuals with severe and persistent behavioral health disorders whose needs are better met with more frequent contact and outreach. This service is a home- and community-based mental health, high-intensity treatment service that provides a multi-disciplinary treatment team approach to assessed individuals in the greatest need of these services. Team members work together to provide intensive coordinated and integrated treatment as described in an individual service plan. Services are designed to rehabilitate individuals who are experiencing severe mental illness symptoms in the community; thereby, a main goal is to avoid more restrictive levels of service such as psychiatric inpatient hospitalization or residential placement and facilitate step down to more traditional outpatient services when ready. Goals of IOP include client reinforcement of safety, promotion of stability and independence of the individual in the community, and restoration and stabilization to a higher level of functioning. The treatment team may be accessed 24 hours a day, seven days a week, often in the client’s home or community, to support individualized treatment needs, ensuring that during a crisis, services are delivered by someone who is directly involved in ongoing care. IOP services are ongoing services and not Emergency Services. The IOP clinicians are the primary program staff responsible for the support of clients in their program.

- Adult Outpatient Services
The Adult Outpatient Program provides a broad spectrum of counseling services to adult clients who have mental health issues. This service gives the assistance clients need to reduce or manage their mental health symptoms by improving their coping and daily living skills. Services include individual, family, and group therapy, as well as community support services. There are many reasons why an adult may seek mental health treatment including those related to depression, anxiety, and mood disorders that impede their quality of life. More severe or debilitating symptoms associated with trauma, psychosis and schizophrenia are also common reasons for someone to seek mental health treatment. Compass Health utilizes a number of evidence-based practices to ensure the best possible outcomes for our clients, including cognitive behavioral therapy, motivational interviewing, and illness management and recovery.

- Children, Youth & Family
Camp Mariposa® Everett is a year-round addiction prevention and mentoring program for youth affected by the substance use disorder of a family member. Youth ages 9-12 attend weekend camps up to six times a year. Weekend camps offer fun, traditional camp activities combined with education and support sessions led by mental health professionals and trained adult mentors.
Additional social activities are offered for youth and their families throughout the year. Many Camp Mariposa® locations offer programs for teens ages 13-17. All program activities are offered free of charge to families. Camp Mariposa® Everett is part of Eluna’s national Camp Mariposa® program.
The camp is led by Compass Health’s Children’s Intensive Services mental health professionals. Compass Health staff and volunteer Mentors provide therapeutic activities in a beautiful camp setting.

- Children, Youth & Family
Child and Family Outpatient services provide an array of counseling and support services to families with children and youth who may have emotional and/or behavioral challenges. Compass Health services are designed to promote positive changes in behavior, help the child /youth and family learn appropriate coping skills, improve communication skills including learning to resolve conflict and manage emotions in a healthy manner. Families seek counseling for a variety of reasons. Sometimes circumstantial life changes can bring about symptoms of depression, anxiety, and mood disorders which impede the child or youth’s everyday life. Youth may have challenges at school, with peers, or in the community. Problems related to traumatic experiences that impact the child, youth and/or their family often initiate the request for counseling in an effort to relieve painful feelings. Compass Health services are tailored to meet the developmental needs of the children and youth and we work to involve family members including foster parents and guardians when that will benefit the client. We offer individual therapy, family therapy, and group therapy using evidenced-based practices as well as case management services to coordinate with schools, Children’s Administration, juvenile justice and other systems that are involved with the particular client. This service is available in Snohomish, Skagit, Island, San Juan & Whatcom counties. WISe is available in Snohomish, Skagit, Island, & San Juan Counties for children/youth up to 20 years of age. WISe TAY (Transition Age Youth) is available in Skagit County & Whatcom County for youth 13-20 years of age. Children’s Intensive Outpatient Services supports children and their families in their community in a least restrictive environment using the Wraparound with Intensive Services (WISe) model. Wraparound works to honor the family voice and choice, identify strengths, build natural supports and foster self-sufficiency. Wraparound teams provide comprehensive behavioral health services and supports to Medicaid-eligible individuals, up to 20 years of age, with complex behavioral health needs and their families. The goal of the program is for eligible youth to live and thrive in their homes and communities, as well as to avoid or reduce disruptive out-of-home placements. Assessment, treatment, and support services are provided in the youth and family’s natural setting, where needs and challenges present themselves (such as the home, school and community). WISe services are available in Snohomish, Skagit, Island, San Juan, and Whatcom (TAY only) counties.

The Community Outreach and Recovery Support Team (CORS) is a specialty program that works with adults in the community who need extra assistance to engage with treatment services and stabilize their housing situation. The team consists of a mix of Peer Counselors and Clinicians, who work in the community to outreach to individuals who would benefit from more intensive treatment services. The CORS team coordinates closely with other Compass Health treatment teams, as well as community partners who serve the same individuals. This service is available in Whatcom county. - Crisis Services
The Crisis Prevention & Intervention Team (CPIT) is now the Mobile Crisis Outreach Team (MCOT) as of 7/1/19. The Mobile Crisis Outreach Team (MCOT) is a community-based outreach team with the ability to respond to and provide crisis services in the community (e.g., homes, schools, shelters). The Team serves adults, adolescents, and children who are experiencing a behavioral health (mental health and/or substance use) crisis regardless of their funding source. MCOT offers short term crisis intervention and prevention services utilizing strength-based, solution-focused, culturally competent, trauma-informed, and recovery-oriented interventions. MCOT consists of a team of behavioral health professionals, including mental health professionals, certified peer counselors, and chemical dependency professionals collaborating with the individual in crisis and their family members to develop a plan to get through the current crisis, manage future crises, and move towards recovery.

Compass Health Crisis Triage utilizes a recovery model to assist those in behavioral health crises to access services and to support and resume typical levels of functioning. Multi-disciplinary teams of Peer Counselors, Mental Health Technicians, Clinicians, Nurses, and Psychiatric providers work together to address needs.
Whatcom County Triage Center
The Whatcom County Triage Center (WCTC) in Bellingham can accommodate 5 individuals who are recovering from a mental health crisis. We provide crisis stabilization, walk-in emergency and next day appointments. Referrals to the WCTC can be made by calling the Crisis Care Line at 800-584-3578. Professionals can call directly to make referrals at 360-676-2020. The mental health facility is co-located with a detox facility that is operated by Pioneer Services, to which individuals needing non-medical sobering support can be referred.
Snohomish County Triage Center
As of April 24, Snohomish County Triage Center is temporarily closed until the re-opening of our new facility as part of our Broadway Campus Redevelopment project. The Broadway Campus Redevelopment project is a commitment to the future of downtown Everett and to our clients – those we serve now and in the future in Snohomish County. This redevelopment will allow us to expand our service capacity and deliver care in a purpose-built facility that reflects dignity for our clients and the level of clinical excellence our teams deliver daily. Lengths of stay at Triage can vary from hours to several days and is dependent on the details of any given crisis episode. Currently, this service is available in Whatcom County only. The Emergency Motel Voucher Program is only available for individuals in Snohomish and Skagit counties in Washington State.
The Emergency Motel Voucher Program (EMVP) is available in Snohomish and Skagit counties and provides short-term assistance to individuals and families in an effort to reduce the number of households who return to emergency shelters.
The goal of the EMVP is to break the cycle of homelessness before it begins by reaching as many individuals as possible and providing intervention in time of crisis. This program allows recipients stability, safety, and the peace of mind required to make a positive change. While a motel voucher is not a permanent solution, it allows our clients a safe place to stay off the streets while our clinicians work with them to find affordable, permanent housing options. Vouchers are good for up to seven days and can be renewed to provide up to 14 days of emergency housing in instances where individuals might be returned to the street to provide more time for placement. Compass Health's standing relationships with local motels allow the organization to quickly house an individual or family. The EMVP allows Compass Health to work collaboratively with not only our own clients, but with other non-profit providers in the community who are facing the same challenges for their own clients. - Inpatient Services
Compass Health's Evaluation and Treatment facility provides a 16-bed inpatient program in Mukilteo to adults with a behavioral health disorders under 120-hour involuntary detention and/or 14-day commitment in accordance with Washington State Involuntary Treatment Act, RCW 71.05. Evaluation begins upon admission to obtain baseline information and to assess physical status, preform psychological evaluation and mental status examination and determine global functioning. The data is continually updated in an individualized treatment plan to provide the highest quality of care possible in a dynamic situation. Clinical outcomes and patient satisfaction are assessed and the data is used to improve treatment within our quality improvement program. Our team of qualified professional staff includes Psychiatric Prescribers, Registered Nurses, Clinicians, Technicians and Peer Counselors who work as a multidisciplinary team to deliver clinical services within a medical model. Registered Nurses are onsite 24 hours a day to provide nursing service functions. Therapeutic didactic groups, medication educations group, recreational therapy, socialization and activities of daily living are offered daily. Our clinicians provide family intervention and support on an ongoing basis. This service is available in Snohomish County. To access these services, individuals must be referred by a Designated Mental Health Professional.

ECS provides behavioral counseling and community support services to eligible clients relocating from State psychiatric hospitals, or who are at risk of going to a hospital. The goal of the treatment team is to allow clients to remain in the community and increase the quality of their life. Clients are usually adults with behavioral challenges that threaten their placements at Adult Family Homes or Skilled Nursing Facilities. We provide, MH OP services, Behavioral Support, and prescribing services. The program is an outreach program including prescribing services. Clients are referred through DSHS - Housing & Community Services and Western State Hospital. Services are in Snohomish county. The ECS team includes mental health professional clinician, psychiatric nurse practitioner and geriatric mental health specialist who provide ongoing services in nursing homes and adult family homes that maintain the individual in their residential setting.

Genoa Healthcare builds on-site full-service pharmacies for the Mental Health Community. They understand the unique and special needs of our clients. A number of services are offered to help improve medication compliance and also allow our doctors to work closely with the pharmacist on medications to keep lines of communication open. Genoa Healthcare pharmacies offer confidential and discrete services to clients and conveniently fill prescriptions.
We are proud to have partnered with Genoa Healthcare since 2004 to provide a full-service pharmacy located in our Bailey Center in Everett, WA with a new site at our McLeod Office in Bellingham. - Healthcare Integration
Health Homes provide integration and coordination of primary and specialty medical care services for those individuals who qualify for this benefit by the Washington State Health Care Authority. Health Homes interventions focus on the following six services: comprehensive care management, care coordination and health promotion, comprehensive transitional care and follow up, patient and family support, referral to community and social support services and use of information technology to link services if applicable. The goal of Health Homes is to achieve improved health outcomes for qualified individuals by actively evaluating needs and strengths and by using an individual's virtues to further their independent pursuit of health and wellness. Health Homes are available in Snohomish, Skagit, Island, San Juan, and Whatcom counties. To access these services, individuals must be referred by the Apple Health Plan.

- Homeless Support & Supported Housing
Compass Health currently provides affordable housing assistance to more than 500 individuals throughout Snohomish, Skagit, and Island Counties. Most of the housing supports those who have a chronic mental illness and are experiencing homelessness. Housing services are available to adults 18 years of age and older who are receiving services; primarily single adults, although there are some resources available for families with children. We are able to provide safe and affordable housing in a variety of ways, through ownership and management of houses and apartment complexes, sponsorship of subsidies such as Shelter Plus Care and Section 8, and collaborations with area Housing Authorities and other housing providers. By providing safe and affordable housing, we are offer individuals an opportunity to focus more fully on symptom management and daily living skills, rather than whether they will be spending the night on the street or in an unhealthy or stressful environment.
Compass Health housing can be accessed through the Coordinated Entry system for each County. Coordinated Entry can be accessed by calling 211. Individuals seeking housing will be required to meet with Coordinated Entry housing resource specialist to begin the referral process. When there is a housing opening, the applicant referral will be made from the County Coordinated Entry candidate pool.

Designated Crisis Responders provide legal assessment of grave disability and/or danger to self or others (as a result of a mental health or substance use disorder) for possible involuntary detention to a behavioral health facility. As a component of the evaluation, it must be determined that other, less restrictive options for stabilization of the crisis have been ruled out or deemed inappropriate. Additionally, it is expected that the investigation include attempts to interview other significant professional and natural supports as well as reviewing client’s behavioral health history to ascertain a more complete picture of client’s current needs. This service is available in Skagit, Island, San Juan, & Whatcom counties. Program of Assertive Community Treatment (PACT)
- Adult Outpatient Services
The Program for Assertive Community Treatment (PACT) is an evidence-based intensive outpatient treatment program designed to assist adults overcome the barriers to their recovery from severe and persistent behavioral health disorders. The program operates with a strength-based and person-centered approach and is targeted to individuals with severe behavioral health disorders who exhibit continuous high service needs, who have experienced multiple inpatient psychiatric hospitalizations, and who have been unable to benefit from more traditional outpatient behavioral healthcare. PACT services are provided by a multi-disciplinary team that includes mental health professionals, a psychiatric prescriber, nurses, a substance use disorder specialist, a vocational specialist, peer counselors, and community support specialists. The entire PACT team carries the caseload rather than an individual clinician. The majority of services are provided in the client’s home or other community settings. Clients are seen several times per week, and sometimes multiple times per day, in order to promote stabilization and recovery. Services include comprehensive assessment, individualized treatment planning, service coordination, counseling and psychotherapy, co-occurring disorder treatment, vocational services, comprehensive community support, peer support, assistance with daily living activities, crisis intervention, and psychiatric evaluation, prescribing, and medication monitoring. Services are provided seven days per week including weekends and holidays in Snohomish and Skagit counties. Crisis services are available to PACT clients from their treatment team 24/7. This service is available in Snohomish & Skagit counties. To access this service, individuals must be referred by a PACT team member, who will submit your referral to the North Sound Behavioral Health Organization.

Psychiatric services are an integrated component of almost all programs at Compass Health. Psychiatrists and Advanced Registered Nurse Practitioners provide evaluations, medication management and medication training/administration for adults, older adults, children and youth. This service is available in Snohomish, Skagit, Island, San Juan, & Whatcom counties. To access these services, individuals start by engaging in another service or program with Compass Health. Your treatment team will work with you to determine if the specialty care of a psychiatric provider should be part of your comprehensive treatment plan.

- Residential Services
Our Residential Treatment Facilities (RTFs) serve adults age 18 and over diagnosed with severe and persistent psychiatric disorders. Services include assessment, psychiatric evaluation, medication monitoring, case management, counseling, groups, mental health education, crisis resolution, and skills training regarding activities of daily living. The program focuses on helping residents reach increasing levels of recovery in order to successfully transition back into the community. Priority placement for residential treatment services is given to individuals discharging from psychiatric inpatient units. Individuals currently living independently but struggling to maintain stability in the community may be referred by their treatment team for this higher level of care. Typical length of stay is between 6-18 months, while individuals focus on developing the skills necessary to live independently in the community. This service is available in Snohomish County. The Compass Health Children's Advocacy Program (CAP) provides services to children and adolescents who have been, or are believed to have been, sexually abused, from birth to 22 years old and their non-offending family members. The youth and family focused services are provided at Compass Health clinics by professionals specifically trained to treat victims of sexual abuse. Counseling and ongoing-therapy are proven to alleviate the long-term conditions that often result from abuse, such as feelings of guilt and shame, extreme anxiety, and depression. Children and adolescents who receive immediate support following the disclosure of abuse learn to deal with these conditions before they establish themselves and become life-long problems. When a parent learns that their child has been the victim of sexual abuse, this can often cause a crisis in the family, accompanied by feelings of guilt and shame, extreme anxiety, and depression for both the non-offending parent and the child. Compass Health’s clinicians are specifically trained to help children, adolescents and their families figure out the most effective way to move past the trauma of sexual abuse and live normal and happy lives. Our specially trained therapists use Trauma Focused- Cognitive Behavioral Therapy (TF-CBT) as their treatment model. This is an evidence-based practice that has been shown many times by research to be a highly effective treatment model for trauma. In addition to our Compass Health clinics, we have CAP specialists located at Dawson Place Child Advocacy Center in downtown Everett.

Compass Health offers outpatient substance use disorder treatment to youth and adults tailored to meet the unique needs of individuals. Services offered include: comprehensive substance use assessment, DUI assessment, outpatient or intensive outpatient treatment, aftercare, random urinalysis screening, case management, as well as coordination and treatment for co-occurring disorders utilizing dialectical behavioral therapy. Treatment services include individual and group counseling. The frequency of individual and group counseling is determined by the level of care assigned by the American Society of Addiction Medicine (ASAM) criteria and can range between monthly, weekly or three times a week. Services may be offered in Spanish to some individuals as needed, or interpreters may be arranged. Services are offered on San Juan Island but are available to anyone that resides on any island in San Juan County. This service is available in San Juan County.
