Adult Family Home In Reach (AIR)

What is AIR?

Adult Family Home In Reach (AIR) provides outreach-based, high-intensity mental health treatment and behavior support for adults with serious behavioral health challenges impacting community living. Services are delivered multiple times per week as needed, often in Adult Family Homes (AFHs) and other community settings.

Who does AIR serve?

AIR serves adults living in Adult Family Homes in Snohomish County who are experiencing intensive mental health and behavioral health challenges. Many individuals supported through AIR have a history of repeated hospitalization, housing instability, and/or homelessness.

Most referrals to AIR are connected to Expanded Community Services (ECS), authorized through Washington State Department of Social and Health Services (DSHS) Home and Community Services (HCS).

Services

Approach to care

AIR uses a team-based, person-centered approach to care focused on recovery, stability, and long-term success in the community. Depending on individual needs, services may involve a behavioral health clinician, peer counselor, medical assistant, and psychiatric provider working collaboratively alongside the client and their support system.

Care coordination may include collaboration with Adult Family Home caregivers, DSHS case managers, medical providers, and other supports, as requested by the client.

Services often begin with ECS behavior support designed to help individuals maintain community stability through caregiver coaching, practical strategies, and individualized behavioral support. AIR may also provide outreach and engagement, therapy and counseling, case management, peer support, skills training, crisis response, and medication management.

AIR supports recovery, stable community housing, and improved quality of life through flexible, outreach-based behavioral health care. Services are designed to help reduce avoidable hospitalizations, homelessness, and involvement with law enforcement while supporting individuals in remaining safely connected to their communities.

Access

Individuals may access AIR through referral and authorization for Expanded Community Services (ECS) by DSHS Home and Community Services (HCS).
