We offer a wide-range of behavioral health services for clients of all ages in Snohomish, Skagit, Island, San Juan, and Whatcom counties.
How to Make a Referral
Follow the steps below to submit a referral for general outpatient services. You can refer by phone or online, then we will reach out to schedule an assessment and complete any required forms together.
We currently accept referrals for individuals with a Washington State Apple Health (Medicaid) Managed Care Organization (MCO) plan with an IMC or BHSO benefit type.
Refer by Phone or Online
You can make a referral for general outpatient services by calling our Access Department or by completing our Online Referral Form.
After You Submit a Referral
An outpatient clinic team member will call you to schedule your assessment.
Complete Client Forms
Your clinical team will work with you to complete required client forms.
