Services

Health Homes provide integration and coordination of primary and specialty medical care services for those individuals who qualify for this benefit by the Washington State Health Care Authority.

Health Homes interventions focus on the following six services: comprehensive care management, care coordination and health promotion, comprehensive transitional care and follow up, patient and family support, referral to community and social support services and use of information technology to link services if applicable.

The goal of Health Homes is to achieve improved health outcomes for qualified individuals by actively evaluating needs and strengths and by using an individual’s virtues to further their independent pursuit of health and wellness.

Health Homes are available in Snohomish, Skagit, Island, San Juan, and Whatcom counties.

To access these services, individuals must be referred by the Apple Health Plan.
